HOW IS REGISTRATION DONE
- Free online registration at www.polytechhealth.com. To register, you need the implant passport, which the surgeon in charge will deliver after the surgery.
- It is possible to register for up to six months after the surgery.
- The registration will only be valid if the online form is completely filled out.
- All documents are sent in digital format, unless postal delivery is requested in the registration process.
- When registering for the Implants of Excellence program, it is necessary to fully accept the terms and conditions presented.
